how do I create the droplist function for dates in outlook appoin.

I am attempting to create a custom form in Outlook 2003. I want to "recreate
the start/end function on a new page. How do I get the droplist to display
the calendar as on the first page?

Outlook itself does not include any date picker control (which, by the way,
is one reason why the first page of the appointment, journal, and task forms
