Hello,
You should design a new form to use as the "New Contact"
form. You can find this feature in the Tools menu of the
standart new contact form. After you design your own form,
you should publish (file>publish form as..) this form
under the folder you want, and in the properties of this
folder, select that form as the default new form. Thats
all.
That applies to all the folders in outlook (mail, journal
and other folders..)
