How do I delete contact forms?

Hello All...

I have successfully created different forms for "Contacts" in Outlook. I've
done this in Outlook 2002 and Outlook 2003. However, in both cases, I've
never been able to delete the forms I create. The "delete" button shows
when I "choose" a form, but it's always greyed out.

Can anyone suggest what action I can take to delete the forms?
