I am using Word 2007.

How do I add controls like Radio button, Check box, lost box to a document?

Have a look for the Developer tab in the ribbon, If it isn't there, click
the Office Button, then Word Options, selct Popular, and check "Show
Developer tab in the Ribbon".

On the Developer tab, you will find various types of Controls (the old
Online Forms controls etc. and the new Content Controls) and realted
buttons in the "Controls" section of the Ribbon

NB, you have landed in a Mac group here, and Word 2007 or an equivalent
isn't on Mac yet, so if you have further questions you would probably be
better off asking them in a group such as microsoft.public.word.programming
