How do I design an authorization form in outlook?

To authorize an action (i.e. credit limit to Joe Smith), my form contains the
necessary fields to send and to read.

On the "read" side, an additional field is included for the approval (i.e. a
"yes/no" field or a text field to enter a quantity.

My question:
How do I make the reply work to include all my user-defined fields in the
reply text?

Currently non of the user-defined fields are included, even with the enabled
"Reply" action set to "include original message text". Instead, once the
authorizer replies to the original form a blank message field becomes
available.
