How do I "forward" field values to subsequent forms?

For form X which contains a field with "Patient Jones" as a drop down
selection and an Optional Section Z within the form, I'd like to take
values entered into the last Optional Section Z filled out for Patient
Jones and have them automatically populate the fields the next time
Optional Section Z is opened in a form for Patient Jones.

How might I do that if it is possible?
