How do I get my custom Outlook Form to display in Web Access?

I have created a custom form in Outlook which I have saved in Public Folders.
I would like everyone in my company to be able to access this form through
Outlook Web Access. However, if I go to OWA and click on the folder in
Public Folders, I

a) do not see the default view that I set up for the folder
b) when I click on New to add a new form, I do not see my custom form format

How do I get my form and my view to display in OWA?

Custom Outlook forms don't work at all in OWA. You'd have to create custom web forms using an entirely different API.

On that folder's Properties dialog, do you have the "create Exchange views" box checked?
