How do I get my"Save and Close" button back on my toolbar?

In Outlook 2003, while adding a new appointment, a strange key press ended up
with the Save and close button being removed from my toolbar on that
appointment. Since then it has disappeared from every New thing I do eg.
contact etc.
I tried to re-add it, as you would do normally to customize the toolbar, but
that command is not there!
Obviously I don't want to have to reinstall Outlook - any ideas how to get
it back?
