How do I get my template applied to my document?

I have made several templates, but I can't get any of my documents to assume
the formatting properties of the template, so I end up doing it manually. I
read in help to open, attach and close and re-open the document, but presto
is "Oh No!"

And note that if you want a document to assume more than the styles of the
template (that is, the document layout--paper size and orientation, margins,
columns, etc.--and/or any text in the template, such as headers/footers),
you must create a new document based on the template and use Insert | File
to insert the existing document.
