How do I have one data source and mutliple forms for input?

We are constanly inputing the same type of data into different office
programs i.e. name, adress and phone. I want to set up one form for input
and depending on a drop down box selection (need help with that too), I want
to populate different office (excel) sheets. Is this possible?
