How do I import a file into a custom form?

I have created a custom form in Outlook to be used as a Task. I want to be
able to use this custom form when I import a file from Access. How do I get
the fields, from this form, to show when I run the import wozard?

You don't. Outlook doesn't support importing to a custom form. You'd have to write custom code or use a third-party application. See http://www.outlookcode.com/d/customimport.htm.
