How do I link email addresses to a check box in an Outlook form?

How do I setup an Outlook form so that I can link multiple email addresses to
a check box? The email addresses will be static address that the form will
be sent to when completed.

I'm having a hard time imagining what you have in mind with regard to linking multiple addresses to a single check box. Or did you mean one check box for each address?
