how do i lock a form to prevent other users making changes

I'd like to create a form that other users cannot alter except to enter their
data to.

An Outlook custom form? You can set a password on the (Properties) page, but it's pretty easy to retrieve. See http://www.outlookcode.com/d/code/getformpassword.htm
