How do I mail merge with Outlook Contacts

When mail merging using Word 2003 and Outlook Contacts, how do I enter first
names only in the greeting line of a letter, e.g. Dear Judy & Chris, yet in
the address block for the envelopes just have the husband's name, e.g. Mr. &
Mrs. Chris Brown?

In step 4 of 6 of the mail merge wizard an option box will open chose Dear
Joshua (4th from the bottom) and in the 3rd dialog box don't forget the comma
or Word will not insert it.
