How do I make my form "go"

I have created a form...saved it in my "personal forms library" chose "run form" from the tools menu item. When I open it in my mailbox...it looks like a regular email (none of my fields are there). Obviously, I am doing something wrong or missing a step or two all together. Can anyone give me info on how to create a form template (start to finish) that is to be enabled by a group of 15 people

The form is a RFQ that our salespeople will fill out and submit to our customer service people instead of filling out paper forms and faxing. Will this even work in Outlook

What is your mail environment -- Exchange or an Internet mail server? Does
everyone use Outlook?
