How do I make transmittals or packaging slips using Outlook?

I want to make packaging slips for plans or specs I send out using contacts
from Outlook. I don't want to keep typing all the information because
sometimes I have 30 packages going out a day. It would help me if I can get
all that done with the help of Outlook contacts. Thanks.

The feature you want to use is mail merge. Select the contacts you want to make labels for, choose Tools | Mail Merge and follow the prompts from there. See http://www.slipstick.com/contacts/printlabels.htm

Actually I kind of figured it out. When in contacts, I selected Action>Send
Letter to Contact. Now the only thing I have to do now is somehow get a fax
template to work with that. I downloaded a fax template from Microsoft but
how would add a macro filed of the contact's name and subject?

That's a different scenario from the one you first posted. If you want a template to appear in the Send Letter dialog, save the template with a file name that includes "Letter." ALso see http://www.slipstick.com/contacts/startletter.htm

The currently open contact is Application.ActiveInspector.CurrentItem. Items selected in a folder are in the Application.ActiveExplorer.Selection collection. Each ContactItem has properties you can look up in the VBA object browser.
