How do I print a custom notecard?

I have a notecard that is 6 1/8 x 4 1/2. It is pre-printed with a design on
the front, and I want to print to the inside right on my HP4050. Whenever I
try to print it, the text is cut off on the top and not aligned correctly.

I have tried changing the paper size, the margins, everything I can think of
and it still doesn't work! I have a huge mailing to do and would appreciate
any help you can give me!
