How do I set up a business development profile of possible client.

I am wanting to set up a profile form of of possible future clients that will
list thier name occupation annual income, and spouses. With also a space for
what min monthly Income they would like to make and how much time willing to
spend per week to obtain that income.also liting thier monthly debts,3 major
goals . Kinda like a short one page prospect profile page of future clients
to determine serious prospects from casual prospects
