How do I set up a form e-mail that will open from hyperlink?

I'm having some trouble - I need to set up a form e-mail that will populate
from a link on a web page. Basically, we want to have the user click and
then have an email open up in outlook (since everyone is required to use
outlook no problem there) with the to field already populated, the subject
line populated and fields that we need to have the user populate in the body
of the message. What is the best way to do this?
