How do I set up a form that can be filled in?

I have a form that I need multiple users to be able to "fill in the blanks"
without changing the layout of the form. I typed the form in Word.
Everytime a user tries to put information in an area the form gets shifted.

Use (border-less) table cells with fixed dimensions to prevent layout
changes as text is entered into the form fields.
