How do I set up a prompt box for name and address in word 2002?

I wish to set up a 'prompt box' in a template letter in order to put name
adress phone number etc to be put in the exact place every time. I have used
similar letters in offices where I have worked so that when you wish to
create a certain letter ie wirh a letterhead as soon as you click on that
letter a box appears with prompts for you to type in eg. reference,name,
title etc. One the info requested is completed it automaticaly places that
info intpo the letter and the rest can be filled in as normal. I have found
it so much easier this way and helps to ensure that the important bits are
included and in the correct place. I have no idea how this is done. I thought
'fields' were the answer but not quite what I was looking for. Can anyone
help me? Windows 2002
