How do I set up a survey series of e-mails, using check boxes?

If anyone can help me, I would so be, like, your best friend forever.....
I work at a museum and we are trying to set up a regular series of e-mail
surveys to our contacts. We'd like to use check boxes. I've figured out how
to insert them in the original draft of the e-mail (Outlook 2003, by the
way). But when I send them, and the recipient replies, the reply message
doesn't have the check-boxes. Help! There must be a way to do this, right?
