How do I set up an Access Database to control a portfolio of acct

I am trying to create a database to control the activity flow of accounts
handled by staff. I need to maintain basic account information, several
account action dates, charges, payments, status of account, and final
disposition, and followup ticklers.

Does any one have a sample, or know of a database I can purchase?
