How do I set up multiple voting buttons?

The same way you set up a single voting button.

If we knew the version of Outlook we could be more specific. The short
generic method assumes you know your way around your version: look on the
options dialog of the compose message window - use voting buttons field.
Type the Vote choices in a semicolon separated list - for example, if we
used voting buttons to figure out what version you used, we'd enter "Outlook
2000;Outlook 2002;Outlook 2003;Outlook 2007" into the field.

Thanks for the quick reply. I am using Outlook 2003 and know how to use
voting buttons for a single question to vote on. What I want to set up is to
have multiple questions and a voting button with a yes or no choice for each
item. Is this possible?

ie.
Do you use sunscreen? yes; no
Do you use baby oil? yes; no
Do you use lemon juice? yes; no
