The same way you set up a single voting button.
If we knew the version of Outlook we could be more specific. The short
generic method assumes you know your way around your version: look on the
options dialog of the compose message window - use voting buttons field.
Type the Vote choices in a semicolon separated list - for example, if we
used voting buttons to figure out what version you used, we'd enter "Outlook
2000;Outlook 2002;Outlook 2003;Outlook 2007" into the field.
