How do I setup custom paper size for newspaper ads?

I must convert my finished .pub document to a .pdf in order for various
newspapers,weeklies to be able to use the file as they cannot work with a
..pub file. I use CutePDF printer to convert. This presents a problem when
my ad exceeds the size of a standard size paper.

I know how to set custom publication size, but I can't find any description
associated with the choices listed in the dropdown for Paper size.
