How do I stop Mail merge from skipping names?

I do a mail merge with 250 names. Everything goes fine except that the
program skips every third name. I've started from scratch many times and it
always does the same thing. Help!

Do you have a Next Record field {Next} in your merge document where none is
required e.g. when trying to merge three records to a page you need one
before the second and third record only?
