How do I type a "Notice of intent to vacate" letter to apt. mgmt?

I bought a new home shortly after signing another 10-month lease agreement.
Now I want to give them a formal "Notice of intent to Vacate" letter, in a
nice proffessional way.
I can"t seem to find the right template here. PLEASE HELP ME?

Since laws may vary state by state, city by city, you need to check with
your attorney or local government agency for the correct wording.

You probably won't find an exact match for your needs and as Tom states,
laws may vary. And if your landlord is anything like other landlords, you're
going to be stuck with some sort of a penalty for breaking the lease.
