How do I type in a plceholder for a name or address in Word 2003?

Can you please help me to figure out how to type in a placeholder in a Word
2003, I am trying to create a template. Thank you so much

Is this for a user-form, mail-merge document or something else (eg a standard letter)?

You should insert text form fields. Address Line 1, Address Line 2, City,
Post Code and so on.
