how do I wite the same letter to a group of people

This is called a form letter and is created using a mail merge. See

I am using microsoft word 2003 and I do not have any of the things this link
is telling me to do. I cannot find the data link and the like. I can actually
do a mail merge but at the greeting section, It is reapeting the name and not
following what I chose for options. Example, I want Dear Mr. John Smith,
however it gives me, Dear John Smith John Smith. I am failing to understand
why it is doing this?
