how do i write a hyperlink to open a specific form?

I would like to send a hyperlink of a form that is
partially complete. e-mail a hyperlink to another person
they hit the hyperlink text that opens the form and this
person will then complete the form.
this is quite easy to just type in the database, however
i want the recipient to go directly to the "active" or
incomplete form...is this possible....?
any help would be greatly appreciated
..
