How do I write an appeal letter to a court?

I am a university student on nonacademic probation and I need to write an
appeal letter to the judicial office in order to get it lifted and removed
from my transcript. What would be the most proper and formal way to write
this letter?

PS: I am a CJ/PLAW student.

I doubt there are any tutorials or templates for such a
specific subject matter. Your typing and grammer seems to
indicate to me that you just have to sit down and compose
the letter based on the facts. Surely, the Dean of your
school has someone that can provide advice on subjects to
be covered.
