How do remove a COM Add-In that is not listed in the Add-In box?

When I open outlook, I get a message that says: C:Documents and
Settings\Mark\Application data\Microsoft\Add-Ins\outlvba.dll. There are two
of these overlaid (you have to click twice to close it. I have gone to the
Add-Inns menu. There is no listing for this add-in, yet when I click the
"COM add-in" box, the same message comes up again.

In the meantime, I cannot send or recieve e-mail. When I click on
"send/recieve," I get an "operation failed" message.

With Outlook closed, try deleting the file
%AppData%\Microsoft\Outlook\extend.dat . If that doesn't work, you'll have
to modify the registry.

Extend.dat applies to Exchange Client Extensions, not COM add-ins.

outlvba.dll is registrered in HKLM, not HKCU, which is why it doesn't show in the COM Add-ins dialog.

What does the error messsage say besides the file name?
