how do you attach a pdf form to a web page

you can right-click on the button or image that you are using as a link and
choose the "hyperlink" option. Then select "a file on your hard disk". Using
the browse function, find the PDF file you want to attach and hit ok. Make
sure you upload the pdf to your web-hosting also.

Would I use these same instructions to link to another index page in another
folder? Basically, I've been trying to link a web gallery I created with
another program. I've tried linking as per instructed, but it's still not
working?

In making the link on the publisher page you go into the hyperlink dialog
box and select the web address option and then enter the URL to the other
web site.

example:

your pub site:

www.mydomain.com/index.htm

your gallery site:

www.mydomain.com/gallery/index.htm

use www.mydomain.com/gallery/index.htm as the link when you create a
hyperlink on the publisher page.
