How do you extend the Office Open / Save As dialogs?

Does anyone know how to extend the Open and/or Save As dialogs that show up
in Office applications? I'm mainly intersted in Word, Excel, and PowerPoint
if it makes a difference.

I have already created an add-in through IDTExtensibility2 with my own menu
item, but I'd rather that my functionality be available directly in the
existing Open and Save As dialogs, if it is possible to extend them.

My technology preference would be to use the .NET 1.1 Framework, but I would
also be interested in hearing about any .NET 2.0 Framework solutions. If the
only solution is unmanaged COM, etc., that's fine (just not preferrred).
