How: Hide Menu Bar and Minimize Buttons

I am able to hide all toolbars BUT I can't get rid of the
menu bar and minimize button on a form.

My goals is to force that user to only be use the command
buttons on the form including the close button to leave
the form.

You can generally set all of what you need in the tools->start-up.

Try running the above. Note how all the ms-access stuff if hidden. To "get
in" and see the settings I used, simply hold down the shift key during
start-up. You can of course disable the shift key if your users start
discovering this by-pass feature.
