How to access hard disk mark as ' Unknown '

On my x225 IBM server, due to a SCSI hard disk corrupted
on my C I have change a new SCSI hard disk. Reload WIN
NT4.0 Server with sp6 and assigned the new hard disk C. As
my second hard disk D is functioning properly, I did not
do anything to it.

On after I reload the OS my D is mark as 'Unknown' drive
and I cannot access the D drive.

Any solutions on how to reactivate my D drive to be
accessible. I do not want to reformt my D. Tks.
