How to add a button toolbar button to run the macro??

I have a mail merge document and have a macro in the
document, it is possible if use the mail merge to create a
new document and also the macro is also copy to the new
document??

or it is possible to add a button in the toolbar and then
by click the button to run the macro, if possible will it
button also appear in the new merged document??

1. Which version of Word do you have?
2. Could you put the button and the macro into a TEMPLATE?
Create a new merge document from the template, and then merge
that. The button should then appear? But does it work?

1. I am using Word 2000
2. Actually I don't know how to add the toolbar button to
