How to add a form/table

I would like to add an additional tier to the project template. The template
currently tracks the project and then the task. I would like to add one more
step and that is the task events. For example, my project is training and
development research, the task is to call XYZ company, I've called three
times and left a message. I would like to track the three calls that I
made...date, comments, status (complete / not complete)

Can anyone tell me how to accomplish this? It sounds easy, but I can't get
it.
