HOW TO ADD FORM FIELDS IN THE HEADER OR FOOTER?

I can do this in the main body of text only. I would like to be able to do
this in the header or footer. Any suggestions?

You cannot do that. You can however insert a cross reference in the header
or footer to the text of the bookmark assigned to a formfield in the body of
the document.

Actually, a cross reference in the header will not work. See the "Headers and
footers - how can I make a form field appear automatic" posting from
9/27/2004.
You have to use a StyleRef, not a cross reference. The posting provides good
instructions.
