How to add toolbar to MS Access from Add-in?

I have tried to add a custom toolbar to Access while add-in is loading
but failed. The "You must have a database open to create custom
toolbars, and the database can't be read-only" message is appeared.
When I should create the toolbar? How to handle the database open
event so that I can add the toolbar?

Any help or comments will be very appreciated!
