how to create a customer feedback database

Hi,
Could anyone help me with a template of a database to manage "customer
feedback". Don't need to be a sofisticated program, just able to create
reports capable of produce data for statistics and detection of trends an hot
issues, etc.

do need Access 2003 to run it.
