How to create a form that I can send online for people to fill out and return

I want to create a form that can be sent to others via email in an
attachment that they can fill out and return by email. Example:
Name_____________________________________________________

Address___________________________________________________

and so on. I want to be able to have people type in the information on
the line without the line
disappearing and keep the original blank form with the lines fixed so
the party filling them out
can't change the basic application content. I have already created 2
forms and just want to convert
them to be used in this mannor. Help!!!!

You haven't mentioned how exactly you made the forms, but I suspect
you just create a line.
Then I would recommend you to use FORMS- that's a WORD feature.
NAvigate to tools>customize>customize menus> and chck forms.
You will see new toolbar now.
Now type in your word document Name and then click on that toolbar on
icon ab|. You will see now grey bar appearing after the word name.
This is FIELD. This says that the recipient is allowed/supposed to
fill his/her name in that grey box. The box stretch itself wile
typing,so don't worry it's too short. To ensure that the recipient
will type only in the grey box,no anywhere in the document I would
recommend you to lock the forms in document - simply click on that
toolbar on the icon with lock. This will lock the document with no
password, but will prevent users to type anywhere else in the document
- word will allow then just to type in the grey boxes.
