How to create a form

Hi and thanks for the help. I am using Outlook 2003. Helps says that
you can create your own forms for data entry but then it doesn't say
how to create forms. Could you give me some direction. I teach Outlook
but don't know about this feature. I want to learn it to teach others
when it is useful.
