HOW TO CREATE A PLEDGE FORM IN MICROSOFT PUBLISHER

I am the media relations director for a baltimore museum. although i've
worked in quark (on the mac) i'm now using microsoft publsiher to create
different forms. for the most part, i'm successful.

our musuem is in the middle of a capital campaign and i am responsible for
"creating" pledge forms. I can't find any that suit me and am wondering if
there's any help "out there" for me.
