How to create a questionnaire

Hello to all members of this forum,

I'm new to this forum and relatively new to Word for Mac. I'm trying to put together a questionnaire to send to my clients and I want it to look as professional as possible.

My question is there a template already installed with Word? do I download one? any suggestions, help would be greatly appreciated.

First you need to decide how you wish to receive your answers.

Then you need to tell us which version of Word you are using.

There's a template here that you might use:

Instructions for using it on a Mac are here:

Thanks for the reply, as for how I want to receive the answers I would like to mail them thus receiving hand written feedback but I would also like to be able to email a digital copy thus receiving electronic feedback. Does that make sense? is that what you were asking?

I'm a Mac user and I just purchased Windows 2008 for Mac. The version # is 12.0.0

Thanks for the templates I'm going to work on them this weekend and see what I can come up with.

Yep, that's what I needed to know :)

My previous answer will be suitable.

I needed to make sure that you did not need the answers to come back
electronically to a database.
