how to create a templates in Visio2003 and add to category

I need to add a template so I can select it when I create a new visio file

I assume you've already built the template? You just want to see it under
the File > New menu, I think?

Go to Tools > Options > Advanced Tab > File Paths... button.

You can enter paths to directories where you store your personal templates.
I think more than one path can be entered by putting a semicolon ";" between
the paths.

Now your templates wil show up under New.
