How to create an index for a master document?

After creating a master document with several subdocuments, I wish to create
an index for the entire master document. I assigned page numbers to each
subdocument including the Chapter number (ie 1-1, 1-2 etc.). When creating
the index, the software picks up all of the marked entries, but ignores the
assigned page numbers, assigning pages in sequence for the entire master
document (although even these numbers are not correct if one counts the
pages). Using Word 2003. I've followed all of the instructions for
pagination for including the Chapter numbers in the pagination. I've created
the index from within the master document as instructed.
