How to create interactive survey form with Office software.

I’m looking to create an Interactive survey template that would use radio
buttons, drop-down boxes, and textboxes to answer some 50 or more questions.

I have been trying to create the survey form using FrontPage, however the
software seems to be web based. I am trying to create a form that would be
accessed/saved on a ...local network.. in either an Excel/Access database.

Would any of the Office programs, for example 'UserForms,' allow me to do
this? If so, which one would best suited for the task?

Hi Student,

MS Office Frontpage is an HTML (web) based product but you can create survey forms in it and data can be 'sent' from a hosted site
to a database. You could create the form in MS Access, somewhat more direct if you're planning on storing the data there as well.

You could also create the form in MS Word or MS Excel. The information here may be helpful.

You may also want to post in either the MS Access, MS Word, MS Excel, or MS FrontPage topical specific newsgroups for better
response. If you're more comfortable working in one app over the other and your responsdents will be able to access the one you
choose (i.e. with Frontpage users would need access to the web page you create not the app) that may help you with your choice.
