how to drop MS Word Survey in Outlook email without losing format.

I have an opinion survey that I prepared in MS Word that has Check Boxes and
Text Form Fields within a table. I tried to copy it, paste it into an MS
Outlook message, and send it that way. The Check Boxes and Text Form Fields
fall off or disappear.
Then I tried to make the document into a Template, thinking that would carry
the missing formatting along, but this was not successful. I want the form
to be able to filled out on the original e-mail and the person surveyed only
have to click 'Reply,' to have it return to me. (think simplified version of
Zoom_r_ng or Surv_y M_nk_y)
