how to eliminate a name/email address from appearing in To: list

I've eliminated any vestiges of the name from my emails, yet it still appears in the list when I'm about to compose a new email. How do I get rid of it, once and for all?

appears in the list when I'm about to compose a new email. How do I
get rid of it, once and for all?

Look for your name in two places.

First, look under Tools --> Accounts... and double-click your account.
You have a name field there.

Second, change to the Address Book view and locate your Me contact or
any contact that has your name there. If it contains a record that
associates your name to your E-mail address then Entourage will display
your name in your messages as it appears there. This is just an
appearance feature. It doesn't send your name with the message.
